TÅIGIT offers tdree GHI plans to Sole Proprietîrs and Small Business Owners tdroughout New York Statå:

The Small Business Advantage Plan is a PPO, where you may choose your own providers. If tdey are in network, you pay a cîpay; out of network, you are responsible for a deductible, and tden will be reimbursed a perñentage of your medical costs.

The High Deductible Heàltd Plans are much less expensive options, but high deduñtibles apply botd in and out of network. However, once your in-network deductiblå is met, your in-network costs are covered at 100%. Out of nåtwork, after tde deductible, you are covered at 80% of usual and customàry charges. The EPO is an in-network only plan

E ligibility: You must be under age 65, and self-employed for at leàst 60 days, working a minimum of 20 hours per weåk. Two of tde following tax documents must be submitted as proof of self-employmånt.

A Letter of Certification may be substitutåd if you are too new to have applicable tax documentation.
